Auto Accidents & Personal Injury
Hurt in a car accident? You may not need to use your health insurance at all. We can bill your auto insurance's Med Pay coverage, the at-fault driver's insurance, or your attorney. Bring your claim number and adjuster's contact information to your first visit, and we'll take care of the rest. Learn more about auto accident care.
Workers' Compensation
Injured on the job? We treat work-related injuries under workers' compensation. Please bring your employer's information and your claim number[, along with your designated provider referral if your employer requires one].
Medicare
We are a Medicare provider. Medicare covers chiropractic adjustments of the spine when they are medically necessary. Some services, such as exams, X-rays, massage and maintenance care, are not covered by Medicare and are billed at our self-pay rates. We'll always let you know ahead of time if a service won't be covered.
HSA & FSA
You can use your Health Savings Account (HSA) or Flexible Spending Account (FSA) card to pay for chiropractic care, including copays, deductibles and self-pay visits.

To keep your visit running smoothly, please have these ready:

Insurance FAQ
What insurance does Optimal Health Chiropractic accept?
We're in-network with Anthem Blue Cross Blue Shield (including out-of-state Blue plans through BlueCard), UnitedHealthcare, Cigna, Aetna and Medicare, and we accept most other insurance plans.
Do you accept Kaiser or Medicaid?
No. Optimal Health Chiropractic does not accept Kaiser or Medicaid (Health First Colorado). Patients with these plans are welcome at our self-pay rates.
How much is a chiropractic adjustment without insurance in Broomfield?
At Optimal Health Chiropractic, a return-visit adjustment is $55 and a new patient exam with your first adjustment is $85. Assisted stretch sessions start at $45 for 30 minutes.
Do I need a referral to see a chiropractor?
Most plans don't require one, but some HMO plans do. If you're not sure, check with your insurance company.
How many visits does my insurance cover?
It depends on your plan. Many plans have a yearly visit limit or require approval after a certain number of visits. Your insurance company can tell you what your plan allows.
What will I pay at each visit?
If you haven't met your deductible yet, most patients pay the full allowed amount for the visit. After that, you'll usually pay a copay or coinsurance.
What do deductible, copay, coinsurance and out-of-pocket maximum mean? These terms decide how much you pay and how much your insurance pays:
-
Deductible: The amount you pay each year for covered care before your insurance starts paying. For most patients, this means paying the full allowed amount for each visit (the discounted rate your insurance has set for that service) until the deductible is met. For example, with a $1,000 deductible you pay the first $1,000 of covered care yourself.
-
Copay: A set amount you pay at each visit, such as $30. Some plans charge only a copay for chiropractic visits, even before the deductible is met.
-
Coinsurance: Your share of the cost after you've met your deductible, shown as a percentage. With 20% coinsurance, you pay 20% of the visit and your insurance pays 80%.
-
Out-of-pocket maximum: The most you'll pay for covered care in a plan year. Once you reach it, your insurance pays 100% of covered services for the rest of the year.
What if my insurance denies a claim?
Our billing team will review it, fix any errors and appeal when appropriate. If a balance becomes your responsibility, we'll explain why and work with you on payment.
Do you bill my insurance for me?
Yes. We submit claims directly to your insurance company, so you don't have to file anything yourself.
